• Certificate of Internship
• Transcript of Records duly certified by Medical School
• College Diploma
• PRC Board Rating
• PRC Certificate of Registration
• PRC License ID (back to back)
• Updated Community Certificate Tax No.
• Latest Trainings/ Seminars attended
• Endorsement Letter from previous residency training (additional for lateral applications only) ​
