The provision of comprehensive, safe and quality nursing care is central to the delivery of quality assurance in patient care provided by the Quezon City General Hospital.
Guided by this vision of the hospital as a center of excellence among local government hospitals in the Philippines, nursing at QCGH upholds to the health care practices and standards set by the organization.
The essence of nursing is in the context of caring, whether at the out-patient department or at the clinical areas. Nursing care is critical to the delivery of high-quality efficient care. Nurses are actively involved in improving both the quality and the efficiency of hospital care. They perform a variety of role in practice, leadership, training, and quality improvement. Thus, they have a strong representation in the different hospital and administrative committee, from the front -liners to the middle managers, to the departmental levels and to the executive level.
WHO expert committee defines the nursing services as the part of the holistic health organization which aim to satisfy the major objective of the nursing service to provide promotion of health and prevention of diseases.
The Nursing Service functions within the concept of shared responsibility and maintains a collegial relationship with the other hospital services. The nurses’ works with the member of other allied discipline such as Dietetics, Medical Social Service, Pharmacy, Rehabilitation, and the like in enhancing the comprehensive program of patient care in the hospital.
Empowerment at all levels is intensified to effectively influence the development of the Service systems and processes.

The scope of Nursing Practice encompasses a 24 hours comprehensive safe and quality nursing care to all patients in a professional manner.

STAFFING FUNCTIONS
The Nursing Service Administrators manage nursing care progress and service for the hospital with a total of 326 nursing complement that includes nurses, midwives, nursing attendants and institutional workers.
Under the leadership of the Chief Nurse, the Nursing Service Standing Committees shall fulfill their tasks and commitment as chair and members of the committees (internal & external) that will lead to quality outcome with near – zero complaint.
INTERNAL COMMITTEES

Examines, evaluate, and verifies patient care services in the context of Nursing Service.
2.Nursing Service Counselling Committee
Receives and reviews occurrences within the service in order to expedite fair adjustment in accordance with the policies provided by the hospital and the Civil Service Commission
3.Nursing Service Promotion and Evaluation Committee
Screen, interviews, evaluates and recommend candidates for promotion to vacant positions of the nursing service
4.Nursing Service Staff Development Committee
Plan for the whole year training program and seminars to upgrade the performance of the nursing staff through the introduction of new concepts, increase knowledge in problem solving and development of good working relations and positive attitude towards work.
5. Continuous Quality Improvement Committee
Reviews, evaluates, revise, and improve nursing care processes and practices directed at both quality and efficiency.

As the hospital continues in its quest for higher quality and better patient safety, there are also nurses and nursing attendants assigned at the endoscopy unit, ECG and EEG unit, TB DOTS and Animal Bite Center.
TRAINING/ORIENTATION PROGRAM
The Nursing Service on its quest for excellence in the delivery of safe and quality nursing care continuously adheres to the requirements of its regulatory bodies in the professional development of its nursing personnel. It has a continuing Professional Development Committee that caters to all in-service and outside training/ seminars of all nursing service personnel. Likewise, it conducts a yearly training needs analysis based on the particular needs of a staff to hone further his/ her skills in the practice of his/her profession.
Regular training sessions are essential for nurses as they help develop and refine their critical thinking skills. This enables them to analyse complex situations, identify potential risks, and make informed choices. Thinking critically is crucial for accurate diagnoses, effective interventions, and ensuring patients’ overall well-being.
Orientation involves introducing newly hired nurses to the unit, providing job -specific training, ensuring consistent training and information about policies and procedures, and familiarizing them with documentation requirements. The Nursing Service has been implementing a structured nursing orientation program for newly hired nursing personnel for several years. The orientation program plays a crucial role in enhancing the competence of new nurses in the hospital setting.

ABOUT US
The Outpatient Department (OPD) of Quezon City General Hospital is a specialized unit that offers diagnostic and therapeutic services to patients who do not require hospitalization. It provides a setting for the treatment of walk-in patients and plays a crucial role in delivering healthcare services at an affordable cost.
In addition to its primary function of patient care, the OPD serves as a training facility for medical residents, interns, clerks, and other paramedical trainees, fostering the development of future healthcare professionals.
The department is responsible for overseeing the delivery of clinical services, including direct patient care and the supervision of clinical teaching activities.
1. Performs document result examination and evaluates this result through the process of clinical reasoning.
2. The standards for practice for patients are met.
3. Collaborates with all appropriate disciplines.
4. Discharges when the goals have been achieved.
5. Intervention is discontinued when the goals have been achieved, patient refuses continuation of intervention, or patient is unable to continue receiving care.
6. Responsible for professional development through any continuing education program.
7. Participates in the education of other allied health professions.
8. Participates in the education of the public, including prevention, health promotion and wellness services.
9. Participate in research activities that provides basis for excellent performance within the scope of practice.

- TB Dots: Consultation and management of pulmonary health issues.
- Pediatrics: Consultation and Care for babies, toddlers and young adults up to 18 years old.
- Ophthalmology: Eye health consultation and treatment.
- ENT (ORL-HNS): Consultation and treatment for ear, nose and throat concerns.
- OB-Gynecology: Consultation and care for pregnant patients (prenatal and postnatal), as well as for female patients with gynecological concerns.
- Surgery: Comprehensive consultation and management for patients who need surgery, including post-operative care and follow-ups for medico-legal cases.
- Dental: Consultation and treatment for teeth, gums and oral health.
- Psychiatry: Consultation, support and treatment for mental health concerns for patients aged 19 and above.
- Internal Medicine: Adult disease prevention, diagnosis and management.
- TB Dots: Consultation and management of pulmonary health issues.
- Family Medicine: Routine check-ups, disease management, health screenings, vaccinations, immunizations, and health education.
Duties and Responsibilities
CHAIRMAN OF THE OUT PATIENT DEPARTMENT
1. The Outpatient Department Chairman shall be responsible for the planning, organizing, directing and overseeing all facilities, activities and concerns.
1.1 Outpatient Department chairman should have constant
communication with all health personnel.
1.2 A regular monthly meeting with the designated personnel of each Department should be held for addressing concerns, for monitoring, updates and future plans.
1.3 Supervises the proper operation of the Outpatient Department. Plan, formulates, implements, direct and evaluates relevant subject matters for all personnel.
1.4 Recommends to the Chief of the Hospital Administrative remedies and procedures for the improvement of the clinical service at the Outpatient Department.
1.5 Attends conferences, seminars, training and convention as needed.
2. Head Nurses
2.1 Supervised the staff in providing quality care to all patients; Managed workflow and ensures smooth flow of work
2.2 Helps the staff in implementing the comprehensive nursing care and delegates task in the absence of Head Nurse
2.3 Acts as a staff nurse in TB-DOTS when needed

Unit Head
- A complete clinical assessment prior to initiation of dialysis and provision of information dialysis and provision of information regarding all renal replacement options.
- Provision of initial dialysis prescription for regular medication.
- Coordination with the surgeon regarding provision and care of patient’s vascular access.
- A document face to face consultation and re-assessment of dialysis prescription and prescribes medication either in the Dialysis unit or in the doctor’s clinic at least twice a month.
- Coordination with the dialysis nurse and/or physician on duty in case of emergencies.
- Long-term care planning, including transplantation whenever appropriate.
- Coordination with other dialysis units and provisions of documentary requirements including Hemodialysis Patients Endorsement (HPEF) in case the patient plans to travel or transfer residence or transfer to the care of another nephrologist.
- Fulfilment of documentary requirements for the patients to avail of benefits from Philhealth and/or other payors.
- Provision of discharge summary/endorsement to attending nephrologist in case of admission under the care of another nephrologist.
Physician on Duty
- Physically present during hours of HDC.
- Attends patients and is responsible for the acute care of the patient undergoing dialysis during his/her duty.
- The care of the individual patients should be in coordination with the attending Nephrologist.
- For hospital-based HDC, the resident in training, hospitalists may perform the duties and responsibilities of the POD, under the supervision of the attending nephrologist of Dialysis Unit Head.
- The residents and hospitalist should have training in the acute care dialysis patient.
- The ratio of POD ratio to the number of HD patient’s stations shall not be more than 1:15.
- Fulfilment of documentary requirements for the patients to avail benefits from Philhealth and/or other payors
- Provision of discharge summary/endorsement to attending nephrologist in case of admission under the care of another nephrologist.
Dialysis Nurse
- Provides direct care to patients undergoing HD based on the rules and regulations established by DOH and approved company policies and procedures.
- Checks and prepares all materials needed and ensures it is available before the start of dialysis.
- Reads and carries out Doctor’s orders and ensures it is performed accordingly.
- Assesses patients using the nursing process before, during and after dialysis.
- Assesses, inspects, regulates and monitors all machine settings and parameters.
- Prepares patient access using required standards for arterio-venous fistula and external access preparations.
- Communicates with the Doctor regarding identified patient-related problems and initiates interventions for the same.
- Administers medications as ordered and properly documents the same.
- Performs, monitors and records pre, intra- and post dialysis treatment vital signs.
- Initiates/performs Cardio-Pulmonary Resuscitation (CPR) in the event of Cardiac and/or pulmonary arrest and sees to it that all activities are supervised, organised and documented.
- Updates patient’s records during and after every dialysis session.
- Inventories e-cart medicines and ward stocks at the end of every shift.
- Protects patients from accidents, hazards, and potential infections.
- Observes the line of communication and applies appropriate communication techniques in reporting, recording and relaying information or data;
- Provides information and health education to patients and significant others regarding the plan of care.
Training
- Attend conventions/ seminars/ conferences related to Hemodialysis and Attended basic life support and Advanced Cardiac Life Support
Activities
- Submit reports regarding monthly census of patients undergoing Hemodialysis.
- Conducts weekly meetings with Unit head in relation to patient cases and review protocols.
