Chief Pharmacist
- Establish the goals and objectives of the hospital pharmacy in coordination with the staff.
- Assure compliance with all legal and regulatory requirements to effectively render hospital pharmacy services (FDA, DDB, PDEA, DOH and other regulatory agencies).
- Develop an organizational structure with clearly defined lines of authority and job responsibilities that enable the staff to work together toward common objectives.
- Instruct, train and supervise employees of the hospital pharmacy.
- Orient pharmacy interns on the Standard Operating Procedures (SOPs) of the pharmacy, hospital organization, and policies.
- Approve pharmacy staff schedule of duties.
- Integrate pharmacy services with other hospital departments and with the medical staff to establish multidisciplinary approach on improving patient care.
- Prepare and submits Project Procurement Management Plan (PPMP) for medicines, supplies and equipment outlay.
- Prepare the pharmacy service operational plan.
- Seek innovative ways to reduce and contain costs while providing quality care.
- Review and update list of pharmaceuticals to be procured.
- Establish policies and procedures to improve pharmacy services.
- Collate reactions/comments concerning prescribed medications and other pharmacy services rendered.
- Report collected reactions/comments to the PTC and Bids and Awards Committee (BAC).
- Approve pharmacy requisitions of supplies and medicines.
- Provide information concerning medicines and drug therapy to all concerned health personnel in the hospital.
- Participate actively as a voting member of the Pharmacy and Therapeutics Committee (PTC).
- Develop a system to assure continuous quality improvement of the pharmacy services.
- Participate in hospital infection control program.
- Assist in the evaluation of Adverse Drug Reaction (ADR) reports, Drug Utilization Evaluation (DUE) and on Infection Control.
- Participate in research activities regarding behavioral, socio-economic pharmaceutical and clinical researches.
- Inspect from time to time, the recorded data in the stock cards.
- Prepare memoranda to the Chief of Hospital, Administrator and other departments of the hospital regarding pharmacy issues and concerns.
- Review and submits financial, statistical, and other required reports.
- Plan suitable educational and training programs for professional staff and pharmacy interns.
- Conduct Continuous Quality Improvement program review of the pharmacy service.
- Participate in the CQI program of the hospital multidisciplinary committee.
- Attend meetings/seminar-workshops related to medicines and hospital pharmacy operations.
- Recommend pharmacy development to the Chief of Hospital.
- Check, evaluate, and approve performance ratings of the pharmacy staff.
- Maintain inventory control program.
- Monitor distribution of dangerous drugs.
- Perform other related functions as instructed by the supervisor.
Assistant to the Chief Pharmacist
- Assume the functions and responsibilities of the chief pharmacist in his/her absence.
- Educate and train pharmacy interns and personnel on the different areas and services of the hospital pharmacy.
- Assist in the planning of activities and in the preparation of policies and procedures on the operations of the hospital pharmacy.
- Supervise clerical and record-keeping activities.
- Collate reports on Adverse Drug Reactions (ADRs), Infection Control and Drug Utilization Evaluation (DUE).
- Supervise the receiving, checking, and proper storage of all drugs, biological products and medical supplies.
- Review schedules and duties of the pharmacists and other personnel.
- Participate in medical rounds and on the review of the patients’ medication history to promote rational drug therapy.
- Supervise inventory taking on pharmaceuticals.
- Prepares list of medicines to be procured and submits the same to the Chief Pharmacist.
- Prepare list of medicines to be posted at the pharmacy-dispensing counter.
- Assist in the planning and preparation of research activities.
- Assist in the preparation of educational pharmacy programs for professional staff and pharmacy interns.
- Supervise/assist pharmacists in the dispensing of pharmaceutical products.
- Assist in the implementation of the CQI program of the hospital pharmacy.
- Attend training/seminar-workshop and consultative meetings related to drugs, hospital pharmacy practices and services.
- Assist the chief pharmacist on the evaluation of the over-all performance of the staff.
Senior Pharmacist
- Assume the functions and responsibilities of the supervising pharmacist in his/her absence.
- Assist the supervising pharmacist in the supervision of Pharmacy Service activities.
- Ensure implementation of the pharmacy SOPs.
- Participate in the inspection of drug products delivered with the Inspection Committee and with the representative from the supply office.
- Guide subordinates in work prioritization and specific tasks to be performed.
- Maintain and update dangerous drugs books,
- Provide drug information to physicians, nurses, allied medical staff and patients.
- Inspect and monitor drug products in all treatment areas.
- Participate in research activities.
- Monitor stocks, equipment, and requisitions regularly.
- Assist in the training of pharmacy interns.
- Participate in the Continuous Quality Improvement Program (CQIP) of the Hospital Pharmacy.
- Perform other related functions as instructed by his/her superiors.
Clinical Pharmacist
- Checks effectiveness of treatment regimens, safety of medications (i.e., drug interactions) and patient compliance issues.
- Expands his/her practice to go beyond distributive functions.
- Reviews and interprets all the medication orders in the patient’s charts during the shift.
- Accomplishes Patient Database.
- Checks Medication and Treatment Record.
- Inserts Pharmacist’s Notes in patient’s chart, e.g., automatic stop order for antibiotics.
- Communicates dug-related concerns to doctors and nurses.
- Report to Antimicrobial Stewardship (AMS) Team meeting.
- Encode antimicrobial use of every patient to Antimicrobial Surveillance Monitoring Logbook.
- Collate the AMS approval form and shall be reviewed by the Antimicrobial Stewardship Committee every three months.
- Assists in coordinating and implementing AMS activities.
- Assists in the development and dissemination of guidelines, monitoring of antimicrobial use and AMR, and in assessing the performance of AMS program.
- Ensures/enforces compliance to all AMS policies, guidelines and procedures.
- Educates pharmacy staff and students on AMS.
- Coordinates with medical and nursing staff to ensure timely administration of appropriate antimicrobials.
- Identifies cases that require review by ID specialists.
Staff Pharmacist
- Fills prescriptions and dispenses pharmaceutical products.
- Checks manufacture and expiration dates of drug products.
- Checks label of medicines in accordance with FDA labeling requirements.
- Records issuances of medicines in the stock cards and accomplishes issue report forms after every shift.
- Files and maintains prescriptions for two (2) years.
- Notifies physicians on any therapeutic incompatibilities, potential drug interactions and duplicate prescriptions,
- Ensures that the right patient takes the right drug and right dosage at the right time.
- Conducts daily/weekly/monthly inventory of medicines and supplies.
- Prepares, check and signs charge slips issued to patients.
- Participate in research activities.
- Attends scientific seminar-workshops/consultative meetings and continuing education on drug-related matters and hospital pharmacy practices.
- Participates in Continuous Quality Improvement (CQI) program of the pharmacy service.
- Prepares Performance Evaluation System (PES).
- Performs other related functions as assigned by the immediate superior.
Storekeeper/Office Aide
- Receives and checks drug product deliveries and record data in the stock cards.
- Records data stocks transferred from the storeroom to the active storage area.
- Encodes, files, receives and releases documents.
- Assis in the daily recording of drug issuances in the stock cards.
- Prepares reports of monthly consumption and balance of stocks.
- Transfers medicines from bulk storeroom to the active dispensing area of the pharmacy.
- Assists in the regular physical inventory of stocks.
- Assists in the inspection of medicines stored in the bulk storeroom as well as in the monitoring of expiration dates.
- Maintains cleanliness of the pharmacy.
- Attends to the administrative needs of the hospital pharmacy staff.
- Participates in the Continuous Quality Improvement Program (CQIP) of the pharmacy service.
- Accomplishes the Performance Evaluation System (PES).
- Performs other related functions as assigned by the superiors.

Pharmacovigilance In-house Lecture Series
- Provision of patient care that optimizes medication therapy and promotes health, wellness, and disease prevention.
- It creates a double check system to the patient, thus, reducing medication errors
- Antimicrobial Stewardship Program was implemented on June 2018 aimed to promote rational and optimal antimicrobial therapy.

Dispensing
- Dispensing of drugs, medicines and medical supplies to customers with the pharmacist exercising his/her knowledge, judgment, skills, controls and ethics to:
- Hospital employees
- Medical staff
- Senior citizen
- Ambulatory patients
- Dispensing of drugs (over-the-counter, prescription and regulated drugs) and medical supplies to in-patients for standing order medications, stat orders and PRN orders.
